Student INTAKE Form Please click here to fill out evaluation forms.  The purpose of filling out these forms is to collect data, which together tells us the number and characteristics of the participants. The data enables us to evaluate whether and how much the IGEN Career Pathways program is improving education, employment, and income outcomes for its participants. Please note that there are two steps to this process.  You will not be allowed access to the Student Evaluation Form until you fill out the Student Intake Form completely. Each form will take about 5-10 minutes to complete. The questions are factual and straightforward. The following FAQ is to inform and answer some of the most common issues and questions asked by students. By RTI.
